Attached is a picture of what the sidebar form should look like. Notice the “privacy policy” and “terms and conditions” links, which aren’t present on the website.
Those links are not part of the widget that you have displayed on the above shared page as shown in the attached screenshots.
To resolve this, you also need to add those links in the widget.

This functionality is developed to load the site faster.
There isn’t any option to disable this functionality but you can set any other image in the widget settings to display in place of the placeholder image as shown in the attached screenshot.

Please see what is displaying for me on the page as shown in the attached screenshot and tell me what you want to do with it or what issue you are facing with it.
This can be your WordPress site issue so to confirm it just temporary use the Twenty Seventeen theme on your site. If the issue still persists then it’s your site issue.
